Submit Identification and Proof of Eligibility for Lifeline Benefits
Identification can be a Drivers License or State Id card, or Passport, or any government issued picture ID.
If you are qualifying based on your participation in a government assistance program such as Medicaid, SNAP (food
stamps), Social Security Income, etc., choose an acceptable form of proof from those listed below.
If you are qualifying based on your total household income, choose an acceptable form of income proof from those listed below.
The proof you submit can be picture, copy, scan, or fax of the document or card, or a computer screen shot of your account that
shows your name, account number and, account status.
Submit the proof by doing one of the following

as well as your full name as it appears on your order. For faxes, write the information at the top of the page.
for emails and text, include the information in the body.
Tribal or Federal participation letter in General Assistance
Child support award
Any official document showing income information over three consecutive month's time.
EBT On-Line Account Links by State
Follow the link for your state below to log into your EBT account and provide us with proof of
Eligibility. If you don't already have an EBT On-line account, you can create one by following the link
below for your state andgoing to the website.
Just send us a screenshot of your account that shows your name and the EBT card status. You can capture a screen shot by
holding down the ALT key and pressing the "Print Screen" key. This will copy a picture of the screen to your computer's
clipboard. Then "paste" into an email to us.
Do NOT send us the page by pulling doing a "File Save As". This will save the HTML markup code and not the data that we'll
need to see.
