Application for Lifeline Cell Phone Service

At the end of the application, you will be asked for:

  • Identification
  • Proof of Eligibility

You should have a picture of, or a pdf of these items ready for upload. If necessary, you can return to the ordering site at a later time to provide them. If you don't provide proof of identity and qualification during the enrollment, you will receive an email with instructions on how to return and complete the application. However, your application cannot be processed without the documentation.

What is acceptable Identification?
Any current government issued identification that has your picture, name, and date of birth on it will be acceptable identification. Drivers License, State ID, and Passport are some examples. You can just snap a picture of the front and back and have those pictures ready to upload at the end of the application.


What is Proof of Eligibility?
In order to qualify to receive the Lifeline Federal subsidy which is what provides your free wireless plan, you must be in a low income household. You can qualify as a low income household by showing evidence that you are enrolled in a qualifying program such as Food Stamps (aka SNAP), Medicaid (not Medicare), Federal Public Housing (section 8), Supplemental Security Income (SSI), and some others. Qualifying programs are listed here.
To prove that you are enrolled in the program, you'll need to upload a picture of the front and back of your program participation card (SNAP or ACCESS card, Medicaid Card, etc), or your award letter, or a screenshot of your on-line EBT account page that shows your name and program. You can find a list of acceptable Proof of Participation documentation here.
You can also qualify based solely on your household income. If you are qualifying based on your income, we'll need some proof of income. Your income for the entire household must be at or below 135% of the Federal Poverty Level.
